TRANSITIONAL KINDERGARTEN AND KINDERGARTEN -

To Enroll:

1. Complete and return the application along with the following:
   • $225.00 Registration Fee/$35.00 New Family Application Fee
   • Copy of Birth Certificate and Social Security card
   • Completed Immunization Record Up to date for TK; Completed for Kindergarten (by first day of school).
   • Kindergarten Health Assessment
   • Medical/Transportation Release Form
   • Permission to Assess
   • Permission to Photograph/Video Form
   • Recommendation Forms - Complete one by church leader and one by family friend and mail to HCA
   • Enrollment Contract
2. After receiving all requested information, the office will contact parents to set up a HCA Board interview with both parents and the applicant(s).
3. Following the interview, parents will be notified about scheduling assessments.  All Kindergarten applicants are required to take the Gesell development assessment. Applicants will be charged a fee of $130.00 on the day of assessment.
4. During the second semester of the Kindergarten year, the students will be assessed for cognitive and academic achievement using the Woodcock Johnson III. A one-time fee of $155.00 will be billed for current HCA students moving to the first grade.
5. Following review of test results by the Admissions Committee, parents will be notified regarding acceptance to the Transitional Kindergarten or Kindergarten program and the enrollment contract will be activated.

GRAMMAR SCHOOL - 1ST - 5TH GRADE

To Enroll:

1. Complete and return the application along with the following:
   • $225.00 Registration Fee/$35.00 New Family Application Fee
   • Copy of Birth Certificate and Social Security card
   • Immunization Record - Up to date
   • Medical/Transportation Release Form
   • Copy of previous school records including any previous testing and all report cards
   • Records Release Form
   • Permission to Assess
   • Permission to Photograph/Video Form
   • Recommendation Forms - Complete one by church leader and one by family friend and mail to HCA
   • Enrollment Contract
2. Upon receipt of application and the above information, the application will be reviewed and a HCA Board interview with both parents and the applicant(s) will be scheduled.

● Former HCA students who are not enrolled for one year may return the following school year without the requirement of entrance testing, provided they supply the school with a copy of the standardized testing done during the year.  Students who are not enrolled at HCA for two or more years will be required to take all entrance testing as a new student.

Logic (6th-8th Grade)/ Rhetoric (9th-12th Grade) Application

LOGIC AND RHETORIC SCHOOL - 6th - 12th Grade

   • Immunization Record - Up to date with booster Tdap (before entering 6th grade)
   • Medical/Transportation Release Form
   • Student Questionnaire - completed and signed by student
   • 9th graders - Copy of 8th grade report card
   • 10th - 12th graders - Copy of high school transcripts
   • Completed Placement Form (from current English & Math Teachers)
   • Permission to Assess Form
   • Permission to Photograph/Video Form
   • Records Release Form
   • Recommendation Forms - complete by one church leader and one family friend and mail to HCA
   • Enrollment Contract
